It is for a baritone; it would also work for a small-receiver trombone. I don't see a manufacturer's identification on it anywhere. It is engraved with the letter "T."
It has a medium deep, rounded cup. The rim (about one-quarter inch) is in excellent condition. The shank end is still well-rounded and undamaged. It looks to me as if this mouthpiece was originally in silver and someone has tried to gold-plate it, and the plating is wearing off in places. Notwithstanding, it is a completely functional mouthpiece and may be of interest to a collector, as it is evidently historical.
